Current Student Annual Checklist
We are so happy to have your child at school again this year! We are required by Licensing to update our records on each child annually. Each year your child attends Angel Care, you will need to complete the forms below. Please have all forms updated and returned to us by September 1st of the current year. If the forms are not returned to us in time, we may be unable to continue care for your child until they are returned. If you have any questions about the forms or how to complete them, we would be more than happy to answer them for you as they arise.
